The Heart Failure Center offers the latest in medical management using nationally recognized guidelines in order to reduce symptoms, improve function, enhance
quality of life and long-term survival. Our expert team led by a board-certified cardiologist and acute care nurse practitioner works closely with referring physicians, patients and their families to evaluate the severity of their disease to develop recommendations for an individualized course of therapy.
This comprehensive program includes office visits and phone calls with an emphasis placed on increasing the patient's knowledge regarding the disease process to allow for eventual self-management. Patients have 24-hour access to the acute care nurse practitioner in order to prevent unnecessary emergency room visits and hospitalizations.
The Heart Failure team may also refer patients to other services such as cardiac rehabilitation, the diabetes, sleep disorder and bariatric centers. For those patients that meet criteria, the team actively seeks referrals for additional treatment options such as device therapy, heart transplantation, and participation in clinical trials.
